This application form is to be used by IT Service Providers to apply for and commence the IT Solution Certification process.

Unless the IT Service Provider undertakes the Self-Certification Assessment and is certified to have successfully complete the aforementioned assessment process, the IT Supplier solution will not be granted connectivity to the Malta Medicines Verification System.
