Onboarding Application

for Wholesalers with a licence issued by the Malta Medicines Authority

This End-User application form is to be used by Wholesalers having a valid license issued by the Malta Medicines Authority , to commence the MaMVS Onboarding process.

Unless the Wholesaler undertakes the Onboarding process by completing the below form and submitting the requested information for assessment and approval by MaMVO, the End-User will not be granted connectivity to the Malta Medicines Verification System.
